This website provides information about our physical therapy, longevity training, and performance testing services, and allows you to contact us and book appointments. Online booking is provided through Jane App, a third-party platform, and your use of the booking site is also subject to Jane's own terms.

A provider-patient relationship with Re.Create is established only when you complete our intake process and receive an evaluation from one of our providers.
Appointments and Payment
Re.Create is a cash-based practice and does not bill insurance directly. Fees, scheduling, and cancellation terms are provided at the time of booking and in your client intake documents. Booking an appointment through our website constitutes agreement to those terms.
