Information you provide directly. When you contact us, book an appointment, or become a client, we may collect your name, email address, phone number, mailing address, date of birth, emergency contact information, payment information, and health and medical history relevant to your care.

Booking and scheduling information. Appointments are scheduled through Jane App, a third-party practice management platform. When you book online, the information you enter is collected and stored by Jane on our behalf. Jane's own privacy policy is available at jane.app/privacy.

Health and wellness data. As part of your care, we may collect assessment results such as body composition measurements, metabolic and VO2 max testing data, force plate testing results, grip strength measurements, and related performance metrics. We may also deliver training programs and collect activity data through Everfit, a third-party training platform.

Information collected automatically. When you visit our website, we and our third-party partners automatically collect certain information about your device and browsing activity, including IP address, browser type, pages visited, time spent on pages, and referring website. This is collected through cookies, pixels, and similar technologies.

Cookies, Pixels, and Advertising Technology

We use the following technologies on our website and booking pages: Google Analytics (helps us understand how visitors use our site so we can improve it), Google Ads (measures the effectiveness of our ads and helps us reach people who may be interested in our services), Meta Pixel (measures ad performance on Facebook and Instagram and shows relevant ads to people who have visited our site), and Google Tag Manager (manages these tracking tools).

We work with third-party platforms that process information on our behalf, including Jane App (scheduling, records, and payments), Everfit (training program delivery), and email and communication tools. These providers are permitted to use your information only to provide services to us.

Advertising and analytics partners. As described above, Google and Meta receive website usage data through cookies and pixels.

Legal requirements. We may disclose information when required by law, court order, or to protect the rights, safety, or property of our clients, our business, or others.

Business transfers. If our practice is ever sold, merged, or reorganized, client information may be transferred as part of that transaction, subject to the same protections described here.

Health Information

We take the confidentiality of your health information seriously. Clinical records are maintained securely in our practice management system and are handled in accordance with applicable state and federal law and the professional standards governing physical therapy practice in Oregon. Your health information is never used for advertising purposes.

Email and Text Communications

If you provide your email address or phone number, we may contact you about your appointments and care. With your consent, we may also send marketing emails such as newsletters or promotions. You can unsubscribe from marketing emails at any time using the link at the bottom of any message, or by contacting us directly. Opting out of marketing does not affect appointment-related communications.

Data Retention

We retain personal and clinical information for as long as needed to provide services and as required by Oregon law governing healthcare record retention. Website analytics data is retained according to the settings of our analytics providers.

Data Security

We use reasonable administrative, technical, and physical safeguards to protect your information, including secure third-party platforms for scheduling, records, and payments. However, no method of transmission or storage is completely secure, and we cannot guarantee absolute security.
